New Patient
• Fill the Pet’s Boarding Form for medication treatment record at CMU Animal Hospital
• Submit the Pet’s Boarding Form to our medical record staff and inform the pet’s body weight to the medical record staff.
• Sit at waiting area and veterinarian will perform a call in queue.
• Pay and receive the bill and/or the next appointment document.
• Submit your receipt at Dispensary / Pharmacy in order to receive medication.

• Inform the name of the animal owner and pet(s) including sickness and the pet's body weight to the medical record staff for medication treatment record.
